Leave (FMLA, OFLA, PFML)
This page will provide all City's employees with an understanding of your rights and responsibilities under state and federal leave laws. Your Human Resources team is here to help navigate the available leave programs.
To apply for leave benefits:
1. Notify your supervisor
For foreseeable leave needs
You must provide the City with at least 30 calendar days notice before taking leave.
In an emergency
You or your designee must give verbal notice to your supervisor within 24 hours of your absence. Written notice to the City is required from you or your emergency contact on record within three days after starting leave.
Using Oregon Paid Family Medical Leave (PFML)?
Under PFML regulations, failure to give notice as required by the PFML statute, may result in PFML benefits reduction of up to 25% of the first week’s benefit.

Complete the packet below that pertains to your need. All items are required unless noted.

3. What's next?
Once the leave request is received, Human Resources will provide you with the following (if applicable):
Initial Notice of Eligibility and Rights
Certification of Health Care Provider
Must be submitted to Human Resources within 15 calendar days after the employer’s request for certification. If the employee fails to provide complete and sufficient medical certification, their leave request may be denied.
Return-to-Work Restrictions Form
If applicable. Must be provided to your supervisor or Human Resources prior to returning to work.

Family Medical Leave Act (FMLA)
FMLA covers up to 12 weeks of leave each benefit year for any combination of family and medical leave for eligible employees or covered family member(s). Leave may be taken consecutively or on an intermittent basis.
You must have worked for the City of Albany for at least 12 months (not necessarily consecutive) and have at least 1,250 actual hours worked with the City during the 12 months immediately preceding the leave.

Oregon Family Leave Act (OFLA)
OFLA covers up to 12 weeks of leave each benefit year. Under certain circumstances, you may be eligible for additional leave for pregnancy-related conditions and sick child leave.
You must have been employed by the City for at least 180 calendar days and also have actual hours worked at an average of at least 25 hours per week during the 180 calendar days immediately preceding the leave.

Oregon Paid Family Medical Leave (PFML)
In addition to FMLA and OFLA, Oregon offers the PFML program administered by The Standard. PFML allows eligible employees to take up to 12 weeks of paid leave in a 52-week period (starting from the day leave begins) for family, medical, or safe leave. If pregnant, you may be eligible for additional leave. Leave may be taken consecutively or on an intermittent basis in increments of work day(s) or work week(s).
You are eligible for PFML benefits immediately upon hire if you have worked in Oregon and earned at least $1,000 in wages in your base year and have contributed to Paid Leave Oregon through payroll deduction in your base year.

Employee Assistance Programs
There are two confidential Employee Assistance Programs available to City employees and their dependents.
SupportLinc
This program provides 8 free sessions (both virtually and or in-person) for employees and their dependents, per existing issue.
The EAP offers a wide range of services, including:
- Counseling Services: Access to confidential counseling sessions for emotional, mental, and behavioral health concerns. Option of in-person, text therapy or virtual.
- Work-Life Balance Support: Assistance with managing stress, work-life balance, and personal responsibilities
- Financial and Legal Advice: Guidance on financial consultation, debt management, and legal issues.
- Coaching: Support for depression, stress, anxiety, or relationship issues.
- Crisis Support: Immediate assistance during personal crises or emergencies.
