When you have a medical emergency it is important responders know about pre-existing medical conditions and current medications. Having this information ensures you receive the best care possible, in the shortest amount of time.
Complete an AFD Emergency Medical Card and post it on your refrigerator. This card provides our responders with critical information such as current medications, physician's name, past surgeries, and emergency contacts. You may also include the location of advanced directives including Do Not Resuscitate (DNR) orders.

This form cannot be used to request medical records as they are not public record.
To request your own medical record for an ambulance transport, you need to schedule an in-person appointment and present your photo ID.
If you are an attorney seeking a record for a client you will need to present an authorization to disclose protected health information signed by the patient.
When Albany Fire Department staff receives the request, they will determine if a report is available, calculate the cost of providing a copy of the report, and contact you with instructions for submitting payment.
The cost varies depending on the size of the report ($7 - $25 average). The fees are allowed under ORS 192.440(3).

AFD offers free car seat inspections by a certified Child Passenger Safety Technician
- Inspections are by appointment only.
- The vehicle the child travels in must be brought to the check.
- Caregivers will be taught to correctly install and use the car seat.

Did the technician determine you need a new car seat?
Thanks to a grant from the Oregon Department of Transportation, and in partnership with the Albany Firefighters Community Assistance Fund (AFFCAF), income-eligible families residing within the city limits of Albany can obtain a car seat at a reduced price.
Car Seat Eligibility
- Proof of residency within the city limits of Albany.
- Meet current WIC income guidelines.
- The car seat technician has determined that a new seat is needed following an assessment, or a referral from WIC.
- There is a $30.00 co-pay per seat, payable in cash upon receipt of the seat.
- Properly fitting seats are distributed only by appointment.
- The child receiving the seat must be present (unless the child is not yet born).
- Limited to one seat per child.
- The program cannot provide infant-only seats. Convertible car seats are well-suited for small babies through the toddler & preschool years.
